Healthy rEQuest is your free wellness program at EMBARQ. You can count on Healthy rEQuest for help, information and advice — log on or call as often as you need. Healthy rEQuest is provided as a benefit by EMBARQ at no cost to you and is completely confidential.

No one at EMBARQ will ever see your personal health information.
 

Healthy rEQuest is administered by independent health management companies — SHPS and StayWell — and is 100% confidential.

Most elements of Healthy rEQuest are provided to all EMBARQ employees, eligible retirees and COBRA participants, but depending on where you work, your job arrangements and other factors, not all of these services may be available to you. EMBARQ intends to continue to provide this coverage; however, EMBARQ reserves the right to change or discontinue any or all benefits at any time without notice. In the event that a benefit is discontinued, EMBARQ will notify participants as soon as reasonable possible.

StayWell

Who is StayWell Health Management?
StayWell Health Management is an independent provider of health promotion programs and services. StayWell specializes in helping organizations improve the health and well-being of their employees. Since 1978 StayWell has designed and managed programs for employers throughout the United States.

Health Assessment (HA)

What is the Health Assessment?
The HA is a confidential, personalized health questionnaire designed to provide information, resources and motivation to improve health and well-being. After completing the HA, you will receive an instant, personalized report that includes ideas and tools to assist in making healthy lifestyle changes.
When can I complete my Health Assessment?
Beginning July 1, 2009 you will be able to log in and complete the HA.  However, you will be required to set up a new user ID and password.
Will I have to enter my screening results into the HA?
No. If you participated in an onsite screening, or if you provide a completed Health Care Provider Form to StayWell prior to completion of your HA, your screening results will be pre-populated; this ensures that your results are based on the most accurate data.
Am I required to complete a health screening and enter my “numbers” to complete the HA?
No; however, you will get a more complete picture of your health if you include it.
Why does the HA ask such personal questions?
Feedback assessing lifestyle habits and health risks is based on how the questions are answered. Age, gender, health history and current health status are all factors that impact HA results. The answers to the questions will help you to identify areas to improve your lifestyle behaviors.

Next Steps®

What is the NextSteps program?
NextSteps is a phone, mail or Web-based program that focuses on lifestyle, is targeted to risk level and is tailored to individual needs. If eligible for these programs, you will receive an invitation to participate in a NextSteps program once you have completed your HA.

Confidentiality

Will my HA results be kept confidential?
Yes, your Health Assessment is completely confidential. All information is treated as confidential personal health information. Your personal results will not be shared with anyone at EMBARQ. StayWell administers the HA to ensure confidentiality and is held to strict privacy and security laws. Individual data may be shared with SHPS, our partner for the Healthy rEQuest programs.  

What if I don’t agree with the Health Assessment privacy statement?
You will need to agree to the HA privacy statement to participate in the HA. If you do not consent, you will not be allowed to complete the HA. StayWell is required by law to receive employee consent prior to obtaining confidential medical information.
