Healthy rEQuest is your free wellness program at EMBARQ. You can count on Healthy rEQuest for help, information and advice — log on or call as often as you need. Healthy rEQuest is provided as a benefit by EMBARQ at no cost to you and is completely confidential.

No one at EMBARQ will ever see your personal health information.
 

Healthy rEQuest is administered by independent health management companies — SHPS and StayWell — and is 100% confidential.

Most elements of Healthy rEQuest are provided to all EMBARQ employees, eligible retirees and COBRA participants, but depending on where you work, your job arrangements and other factors, not all of these services may be available to you. EMBARQ intends to continue to provide this coverage; however, EMBARQ reserves the right to change or discontinue any or all benefits at any time without notice. In the event that a benefit is discontinued, EMBARQ will notify participants as soon as reasonable possible.

Read all about your free Health Assessment

Starting July 1, 2009, visit www.myhealthyrequest.com to complete your HA. Just log in and follow the instructions. As a result of some Website enhancements, you will be required to set up a new user ID and password.

The HA provides quick information about your health — conveniently and free of charge. Your responses will reveal instant information about your health behaviors, so you can make choices about what lifestyle changes, if any, are needed to be your healthiest.

To have a more complete picture of your health, it is recommended to “know your numbers” (blood pressure, glucose, total cholesterol, HDL, height and weight) and enter them in when you take your HA. For active employees, if you participated in an onsite health screening, StayWell will automatically load this information to your record.

Your spouse or qualified domestic partner also is invited and encouraged to complete an online Health Assessment if he or she is enrolled in an EMBARQ-sponsored medical plan. For more accurate information, he or she should have a biometric screening independently so that data can be included when completing the online Health Assessment.

If you do not have your numbers, attached is a Health Care Provider Form that you can print and take to your physician for completion. Then use this form when completing your HA. Your spouse also can use this form. You are responsible for any charges not paid for by your medical plan.
